Unfortunately accidents happen. An important feature of your membership to North Dakota Soccer Association is the option to file a claim with us and have medical expenses relieved. When you sign up with a member club of North Dakota Soccer Association, you are signing up for insurance as well. Below is the link that provides step by step process to file a claim.
Concussion Forms
USYS has provided a form for clubs to fill out if an concussion take place during a USYS event. We strongly encourage your participation. In addition, USYS has provided concussion education information as well.

Tournament Hosting and/or Attending Forms
Any club looking to host a tournament within a North Dakota border must complete full tournament paperwork. To apply follow these steps:
1. Complete United States Youth Soccer Application: USYS Application to host Tournament or Game
2,. Review, print and design your tournament rules by the guidence of the following link: Rules Checklist for Hosting a Game or Tournament Involving Foreign Teams
3. Complete USSF International Tournament Hosting Application
4. Complete USSF International Tournament Hosting Agreement
5. Provide list of all teams (excel version) attending the tournament no later than 1 week prior to the event
